Care Physician

faxed
to our
Provider
Relations
Department,

a Primary
Care
Physician?
A: Primary
Care can
be established
near your
home.

get
an authorization
for
a Specialty
Care
Clinic?
A: In
order
to be
seen in
any of
our Specialty
Care Clinics
you must
have an
authorization
form from
Your Primary
Care Physician.
If you
are seen
in any
Emergency
Room at
any hospital,
please
be sure
to contact
your Primary
Care Physician
to obtain
authorization
prior
to being
seen in
our clinics.
In the
event
that you
do not
obtain
authorization,
you may
be rescheduled
or redirected
to the
proper
Specialist.
